MANAGEMENT OF COMPLEX FISTULA IN ANO BY KSHAR SUTRA TREATMENT

Case History
C E M.R fistulogram was done taking multiplanar & multisequential imaging on 1.5 Testa Superconducting magnet (Siemens Avanto-18 channel system) using 10 cc of Inj. MAGNILEK intravenously. No adverse reaction was noticed during or after the administration of contrast medium.
Findings:-
Two external perianal openings are seen at 3 0′ clock & 5 0′ clock positions. An enhancing linear tract from 5 0′ clock external opening is seen extending antero – superiorly in the soft tissues for a distance of approx. 5.2 cm & then joining the 2″d tract coming from 3 0′ clock opening Single common tract formed at 5-6 0′ clock position is opening into the anal canal in midline at 6 0′ clock position.
Urinary bladder is normal contour. No diverticulum or calculus is seen. No free fluid seen in the pelvis. Major vessels of pelvis appear normal. Both psoas muscles are normal. Visualized bones are normal.
Impression:
FINIDINGS ARE SUGGESTIVE OF POSTERIOR TRANSSPHINCTERIC FISTULA WITH SINGLE INTERNAL OPENING AT 6 0′ CLOCK POSITION & BRANCHING OF THE TRACT VITH TWO EXTERNAL OPENINGS IN THE PERINEUM AT 3 0′ & 5 o’ CLOCK POSITIONS.
